Diet and lifestyle guidelines for prevention of cardiovascular disease and diabetes amog elderly persons

Abstract

Elderly subjects have unique nutrient needs with revised recommended dietary reference intakes based on age, sex, ethnicity and special vulnerability to compromised nutrient status. The food intake tends to decrease with advancing age due to decreased physical activity and metabolic rate. However vitamin; A, E, C, B vitamins and beta-carotine and minerals magnesium, calcium, selenium, chromium, zinc and copper as well as w-3 fatty acids needs either remain constant or increase. Omega-3 fatty acid, calcium, vitamin D and magnesium and antioxidant intakes are inversely associated with ageing and other complications like cardiovascular diseases, diabetes and dementia as well as cancers. It is pertinent to advise to eat 400 g/day of fruits, vegetables and nuts and another 400 g/day of whole grains in conjunction with 25-40 g/day of canola oil or mustered oil, depending upon energy requirement, for prevention of cardiovascular disease and type 2 diabetes mellitus.

Visualization of amplitude-phase relationships in entrainment processes

Abstract

In the present research the visualization of amplitude-phase relationships in entrainment processes is based on the expression serial time sections [3] allow tracing the behavior of different parameters of any rhythmical component of spectrum during a time which includes spans before, during and after any synchronizing (desynchronizing) event. There is a simple and noninvasive method for acquiring information on the visualization of amplitude-phase relationships. The method used the mathematical and graphic analysis simultaneously visualization the dynamics of amplitude, its statistical significance and acrophase in entrainment processes, the three modifications of serial sections was elaborated: procedure of adding logarithms of P-values-matrices, plotting 2D image and 3D image. As examples for illustration serial section method, automatic measurements of blood pressure (BP), both systolic (S) and diastolic (D), and heart rate (HR) were used. Five flights from Minneapolis (USA) to Saint-Petersburg (Russia) crossing 9 time zones and 4 flights in the opposite direction were performed during that time (during the second return flight, no measurements were taken). The simultaneous drop in amplitude and the rapid shift in acrophase associated with West-to-East flights, and more gradual changes for East-to-the-West flights led to the hypothesis that the rapid entrainment is performed by collapse of the rhythm at a singularity time point. Application of the present method has allowed the authors to carry out visualised dynamic chronobiological the control parameters of cardiovascular system. The method has higher diagnostic value in comparison with well-known procedures of an estimation of chronomedicine.

The polymorphism c677t of the gene MTHFR correlates with the complications of the coronary heart disease and arterial hypertension for the patients with dislipidaemia

Abstract

The present study was designed to determine the possibility of the application the genotyping of the patients with dislipidaemia as an additional factor for the prediction of the cardiovascular complications. The distribution of the polymorphism C677T of the gene MTHFR among the 62 patients with dislipidaemia was studied. Allele-specific PCR was applied for detection the C677T mutation/ These results provide the genetic evidence that 677T allele of the gene MTHFR correlates with the development of the complications of the coronary heart disease for the patients with dislipidaemia (p = 0,01). It was demonstrated that more complex and durable antihypertension and cardiotropic therapy must be applicated for the carries of the 677T allele among the dislipidaemia patients. These data indicate the appreciable role C677T polymorphism of MTHFR for the decreasing of the cardiovascular complications among the patients with dislipidaemia and for detection the complexity and longevity of the medical treatments.

Air ions and human environment

Abstract

Conformity to hygienic air ion formula of working offices, classrooms and chemistry laboratory in which the work with volatile organic solvents are conducted, as well as to a city apartment and a country house is investigated. Concentration of air ions was measured using the counter «Sapphire 3М». Parameters of the air ionization: concentration of ions of both charges (N and N-, ions/cm3) and unipolarity coefficient (K = N/N-) were evaluated. The quantity of air ions did not correspond to sanitary and hygienic standards in all areas except of a country house. Additional artificial air ionization and ventilation in classrooms and chemistry laboratories offered to use.

Blood preserving technology in cardiac surgery

Abstract

According to the program of blooding surgery 297 patients were operated with coronary heart disease, acquired and congenital cardiac diseases and others. The introduction of principles of bloodless surgery is favoured by not only risk from donor transfusion, but also by the results of the reserchers dealing with the body's adaptation to acute anemia. The present study was undertaken to make a scientific-and-practical assessment of actual own blood funds and their introduction in order to decrease or refuse to use donor blood at cardiac surgery under extracorporeal circulation. Own blood saving methods, such as acute normovolemic hemodilution, washed autoerythrocytic reinfusion, autoplasma reinfusion, become affective in reducing hemorrhage after extracorporeal circulation. This method is safe, simple in organization, and permitted to avoid allogeneic blood transfusion and its complications.

Functional activity of platelets in allotransplantation of the hypothalamic-pituitary-gonadal axis

Abstract

The research representsthe investigation and implementation in the clinic immunosupressive therapy after allotransplantation of the hypothalamic-pituitary-gonadal axis. Patients received immunosupressive therapy (prednisolone, choriogonin, heparin, a local X-ray irradiation) after organ transplantation. Immunosuppressants were cancelled stepwise. The results showed that the basical immunosupressive therapy provides stable acceptance of allotransplants of the hypothalamic-pituitary-gonadal axis.

Abstract

Paper is devoted to the safety rules during surgical interventions on patients with permanent pacemaker. Data of 123 operated patients with intraoperative pacemaker instability were analyzed; possible causes of these complications were discussed. The use of monopolar electrocoagulation proved to be the most frequent cause of pacemaker failure. Strong correlation between rhythm failure and duration of the single electrocautery exposure was revealed.

The impact of perioperative oral supplementation with immunomodulating diets on postoperative period in patients with major gastrointestinal surgery

Perioperative administration is an important period for nutritional status management. The aim of the study was to assess the impact of perioperative oral immunomodulating diets supplementation in 41 patients after elective major gastrointestinal surgery. Perioperative oral immunomodulating diets supplementation with glutamine and Ω-3 fatty acids during 5 days before surgery enchanced the immune status of the patients,reduced the duration of ICU and hospital stay, and decreased the incidence of postoperative infectious complications.

Perioperative prevention of cardiovascular complications in patients with atherosclerotic lesions of the infrarenal aorta

Abstract

The study involved 237 patients aged 54 to 69 years who underwent aorto-bifemoral bypass surgery due to the atherosclerosis of aorta and its branches. Depending on a method of the anesthesia, patients were divided into 3 groups: in 1st group NLA was used, in the 2nd - epidural anesthesia at ThXII-LI with mechanical ventilation and in the 3rd - thoracic epidural anesthesia at ThX-ThXI with mechanical ventilation. The patient groups were representative. The most high and stable rates of coronary blood flow and central hemodynamics were in patients with thoracic epidural anesthesia (ThX-ThXI) with mechanical ventilation, due to the small doses of local anesthetic combined with a narcotic analgesic. Blockade of the sympathetic trunk has provided the most high and stable parameters of coronary flow, stroke output index and cardiac output fraction compared with other groups of patients.

It was determined, that among SNPs of 7 human genes only allelic variants of IL-1β, IL-28B, TGF-β1 and viral genotypes can influence on probability to achieve SVR.

Assessment of endothelial function in children and adolescents with arterial hypertension by high-resolution ultrasound

Abstract

In 72 children and adolescents with arterial hypertension I-II st. after reactive hyperemia using high resolution ultrasound lower rates of increase in the diameter of the brachial artery (BA) and peak blood flow velocity were detected compared to the normotensive control group of children that may be regarded as the depletion of vasodilatation. In children and adolescents with hypertension during occlusion test а more significant decrease of the diameter of BA was revealed, indicating the functional arteriolar hypertonicity, as well as the possible remodeling of arterial vessels. These results should be interpreted as increased tone of BA in children and adolescents with hypertension.

AGE-specificity in acute pain syndrome measurement in otorhinolaryngology

Abstract

In this study, 133 patients were divided by age into three groups. First group: from 17 to 29 years old; second group: from 30 to 49 years old; and third group: from 50 to 75 years old. Pain intensity of patients was measured by means of a Visual Analogue Scale (VAS), of a Numeric Rating Scale (NRS) and of a Verbal Lightning Scale (LS). All three scales could be used in measurement of pain of patients of the first group whereas NRS is preferable for the second group and LS - for the third group.

Clinical efficacy and safety of Ceraxon® for treatment of primary open-angle glaucoma

Abstract

35 patients with primary open-angle glaucoma (POAG) with unstabilized clinical course and normalized intraocular pressure were treated. All patients were divided into subgroups: 8 patients - with the Ist stage of POAG, 12 - with the IInd and 15 with stage III-IV. Patients were examined with clinical-ophthalmologic and instrumental methods. After the treatment by CERAXON® an improvement of visual functions were noted, accompanied by significant positive dynamics of perimetric and morphometric characteristics: average light sensitivity and defect, dispersion, average thickness of retinal nerve fiber layer. During the treatment by CERAXON® there wasn't any case of side effects, demanding drug withdrawal or change of therapeutic regimen.

The influence of efferent-quantum methods on the immunity in patients with psoriasis

Abstract

Aim: to study the influence of plasmapheresis, ultraviolet blood irradiation and their combination on the immunity indices. 142 patients with the progressive stage of extensive psoriasis from the age of 18 to 67 have been investigated. Indices of phagocytosis, cellular and humoral immunity have been studied in dynamics. Elevated levels of immunoglobulin M and A, circulating immune complexes, an imbalance of T-cell immunity as well as increasing neutrophil activity level of protection were revealed. The inclusion of the efferent-quantum methods in the complex treatment of psoriasis contributes to the normalization of T-lymphocyte subpopulations and the absorptive activity of neutrophils, reducing the CEC, immunoglobulin M and A. Better immunological efficacy noted when plasmapheresis and plasmapheresis were used in combination with ultraviolet blood irradiation.

Diagnosis aggregation aspirin resistance in patients after myocardial infarction in the secondary prevention of thrombotic complications

Abstract

The use of acetylsalicylic acid as antiaggregant is the standard antithrombotic therapy for secondary prevention in patients with myocardial infarction. But the methods of monitoring and analysis of cause of variable efficiency of this therapy is not currently specified. The purpose of this study was to evaluate platelet aggregation activity in patients with myocardial infarction and analysis of the causes of aggregation aspirin resistance. We examined 79 patients after myocardial infarction receiving aspirin. Presented the characteristics of aggregation curves corresponding to an effective antiplatelet therapy. It was revealed that the development of aggregation aspirin resistance depends on the presence of concomitant diseases, as well as receiving beta-blockers and diuretics, and does not depend on the concentration of platelets and acetylsalicylic acid in the blood.

Analysis of domiciliary deaths of urban population

Abstract

The study presents the results of the analysis of domiciliary deaths of the population of a municipal polyclinic in Ufa in the dynamics between 2001 and 2010. The differences in the structure of mortality to the cause of death and sex between working-age population and pensioners were revealed. In ten years increased mortality from digestive system diseases (2,1 times) and malignant tumors (11,5%) was registered. For 2010 the mean age of the dead by months, days and time of death was detected. The obtained results were used as a basis for administrative decision making.
