Perinatal outcomes and preterm birth risk
Abstract
Questions about the method of delivery in incomplete pregnancy always remained and would remain relevant. One of the main causes of perinatal mortality in preterm infants is a generic injuries, largely due to inadequate maintenance of preterm birth. Selection of the optimum tactics of perinatal risk factors — the most important reserve for improving the health status of newborns.

Risk factor of recurrent fetal growth with restiction
Abstract
The case-control study was designed to determine prevalence and risk factors of fetal growth restriction (FGR) in 588 women. Two hundred fifty three factors were analyzed. It’s shown, that FGR history 6.1-fold increases the risk of FGR in consecutive pregnancy (OR 6.1; CI 95% 3.7—10.0; p < 0.001). Main FGR risk factors are obesity (OR 7.6; CI 95% 2.0—29.2; p = 0.037), more than two cases of early pregnancy loss in history (OR 3.2; CI 95% 1.9—9.2; p = 0.037), chronic endometritis (OR 4.1; CI 95% 1.6—10.6; p = 0.006).

IVF performance and complications of I trimester of pregnancy the extent IVF in women with latent iron deficiency
Abstract
We studied early pregnancy complications in pregnancies following in vitro fertilization. The trial included 140 women who were divided into groups according to the presence of latent iron deficiency. In patients with latent iron deficiency frequency of threatened miscarriage and early reproductive loss, as well as the frequency of bacterial vaginosis was significantly higher. In addition, the clinical pregnancy rate was significantly lower in women with latent iron deficiency.

Clinical features of early pregnancy and magnesium content of the blood serum and erythrocytes in women with undifferentiated connective tissue dysplasia
Abstract
A prospective study found that early pregnancy in women with undifferentiated connective tissue dysplasia is characterized by a higher frequency of complications. А significantly lower magnesium content in erythrocytes of pregnant women with dysplasia was determined. The decrease of magnesium in red blood cells in women with dysplasia less than 1.65 mmol/L can be used as a predictor of the risk of retrohorial hematoma.

Gestational weight galns in the first trimesrter and risk of pathological birth weight deviations
Abstract
The cohort study of 467 pregnant women. Analysis has indicated impact of body weight gains in the first trimester on antenatal fetal growth. Low body weight gain or weight loss in early pregnancy were associated with increased risk of small for gestational age. And on the contrary, a fast weight gain with accumulation of a fatty component was associated with the subsequent macrosomia.

3D Doppler ultrasound of placental blood flow in the prediction of a missed miscarriage
Abstract
Reduced placental vasculature was revealed using 3D Doppler ultrasound at 8—14 weeks' gestation in 25 women who had missed miscarriages in 2—6 weeks. The values of placental vascularization index below 10,8% and vascularization flow index less than 3,6 at 8—14 weeks' gestation are predictors of missed miscarriages and early placental insufficiency.

The value of endothelial dysfunction markers in the pathogenesis of morphological and functional changes of chorion in women with previous reproductive loss
Abstract
The value of the markers of endothelial dysfunction in the pathogenesis of morphofunctional changes chorion in women with early reproductive losses. The principal causes of intravascular cytotrophoblast invasion insufficiency are: reduction of NOs level and E-celektine level in serum samples and a rise in Endothelin-1 levels, which correlates with the changes of circulating population of platelets — depressed numbers of platelets at rest, rising of quantity of degenerated platelets which leads to a low vascular resistance in the uterine, spiral and radial arteries. Definition of markers of endothelial dysfunctions, changes in parameters of the morpho-functional states of platelets, increase in the quantity of degenerated platelets and ultrasonic estimation of the blood flow in the uterine, spiral and radial arteries may serve as prognostic criteria to determine the end result of early pregnancy losses.

Microbial colonization and nosocomial infections in preterm infants in the neurosurgical hospital
Abstract
Work is devoted to studying of microbial colonization and infection associated with health care in preterm infants with severe intraventricular hemorrhage (IVH) to evaluate the formation of antibiotic-resistant strains and the choice of treatment. In preterm with IVH complicated by progressive hydrocephalus revealed different variations of the colonization of hospital microorganisms, which changed over hospital treatment. The main microorganisms, which colonized mucosa of oropharynx, respiratory tract and intestines, were Gram negative bacteria multiresistant to antibiotics. In patients receiving long-term antibiotic therapy (1,5 months or more), despite the use of antifungal drugs, appeared fungal flora; in 15% of cases — types of Candida non-albicans. The cause of nosocomial ventriculitis in infants with post-hemorrhagic hydrocephalus in 70% had Gram positive organisms, in 30% — combined bacterial and fungal infection. When choosing a treatment strategy for these children must take into account the results of microbiological studies and antibiogram.

Predicting risk of congenital malformations the fetus at the planning stage of pregnancy and early prenatal period
Abstract
The authors constructed a model by the method of binary logistic regression, which allows to predict the occurrence of congenital malformation of the fetus depending on the presence of risk factors in women, preparing for pregnancy. The proportion of true predictions using the regression model was 76,5%.

The features of mesenchimal stromal cells of normal endometrium and endometrium from patients with different gynecological diseases
Abstract
This is the only experimental study that offers mechanical isolation of stromal cells from normal endometrium and endometrium from patients with adenomyosis, uterine myoma and endometrial hyperplasia. Endometrial stromal cells have been cultured and characterized. And there are comparative data on the expression of surface markers, which are phenotypic features of mesenchimal stem cells, by endometrial stromal cells.

Molecular and cellular determinants in the diagnosis endometrial hyperplasia
Abstract
We analyzed clinic morphological peculiarities of hyperplastic processes in endometrium of patients during menopausal period and educed molecular and cellular nosotropic determinants of illness; presented evaluation of efficiency of immunihistochemistry of biomolecular markers of proliferation and apoptosis (Ki67, р27, р53 и cyclin E), and also receptor status of endometrium. The author detected relationship between the contents of markers and morphological form of hyperplastic process.

The role of pregravid preparation in reproductive losses decrease at patients with a hyperprolactinaemia
Abstract
Patients with infertility and hyperprolaktinaemia have early reproductive losses in 27,4% of cases after pregnancy in a natural cycle, in 33,7% of cases — after the assisted reproductive technologies (ART) programs. Programs of a pregravid preparation developed considering the individual risk factors, allows to reduce the frequency of reproductive losses to 5,8%, premature births — to 2,9%.

Underweight and gynecological disorders young women
Abstract
The results of kogornogo prospective study on the prevalence of underweight, frequency and structure of gynecological diseases in women 20—25 years old. Found that the prevalence of underweight in this cohort of 122,61‰, the prevalence of gynecological diseases — 489,83‰. They predominate inflammatory disease (47,9%), menstrual disorders (25,7%) and non-inflammatory disease of the cervix (20,3%). There is evidence that women with underweight incidence of gynecological diseases is 1,5 times higher compared to normovesnymi peers, low body weight in this cohort have a negative impact on the incidence of dysmenorrhea and menstrual disorders, as well as a risk factor for endometriosis. For women with underweight typical late menarche, slowing the formation of menstruation at puberty is reached reproductive age — the menstrual cycle on postponiruyuschemu type and high frequency of irregular and prolonged menstruation.

New ways to overcome infertility problem in women of advanced reproductive age
Abstract
Assisted reproductive technologies (ART) efficacy directly depends on patient reproductive age. “Poor” ovary response, diminished quality of oocytes and embryos obtained and as a result decreased ability of blastocyst nidation to decidua are obstacles to good results of ART program. Melatonin, a neuropeptide, is well-known to be deeply involved in the process of organism ageing. This hormone takes part in oxidative stress degree decreasing in ovary follicles and defends oocytes from aggression of free radicals. The aim of this study was to assess the outcome of traditional ART program with additional correction with exogenic melatonin in infertile women of advanced reproductive age. It was characteristic for infertile patients of advanced reproductive age to have sharp decrease in levels of melatonin secretion. This reflects disturbances of hypothalamo-pituitary chain of reproductive system. Additional donation of exogenic melatonin (Melaxen) in some cases of ART program potentially increases the chances of successful outcome.

The peculiarities of treatment of cervical diseases associated with HPV-infections in teenag and young girls
Abstract
The differential treatment has been worked up according to the form of ectopy and viral load and based on the study of colposcopic, cytologic and immunologic findings. The effectiveness of isolated immunomodulating therapy in case of uncomplicated cervical ectopy is 75,5%. The using of immunomodulators in complex therapy combined with radio coagulation allows to improve the effectiveness of treatment from 66% to 98,5% in the group of patients with complicated cervical ectopy if compared with the group where immunomodulators have not been taken.

Quality of life of postmenopausal women with pelvic dysfunctions
Abstract
Nowadays, it is clear that pelvic dysfunction is not associated with a significant risk to life. However, this issue has a significant impact on quality of life (QOL) of patients, leading to its degradation (physical, psychological, sexual, and family well-being). In our study we examined 274 postmenopausal women with pelvic dysfunctions, who were distributed in two groups: in I group included 145 patients (52,9%) who underwent surgical repair of pelvic dysfunction by traditional methods, II group contained 129 patients (47,1%) with the use of synthetic materials. Women were inquired by Quality of Life’s questionnaire «Short Form 36 Health Quality Survey» (SF-36) before and after surgical management. The results of the study showed that the quality of life in women after surgery improved better in group I than in group II.

Immunihistochemical analyses of collagen I and III and elastin in uteri ligaments in female genital prolapse
Abstract
The frequency of the female genital prolapse varies from 34,1% to 56,3% in different countries. Pathogenesis and morphological changes in uteri ligaments in female genital prolapse are not clear and have to be investigated. The goal of our study was to investigate uteri ligaments in female genital prolapse by immunohistochemical analyses of collagen I and III and elastin. The study was performed on biopsies of sacro-uterine and round ligaments from 15 women with genital prolapse and 15 women without prolapse. The tissues were fixed in formalin, embedded in paraffin and cut for the slide. Immunohistochemistry was done with primary antibodies collagen I and III (Novus Biologicus, 1:100 и 1:200 and elastin (Thermo Scientific, 1:100). Morphological analyses of ligaments in prolapse showed fibrosis and elastic fibers fragmentation. Immunohistochemical analyses revealed collagens disbalance: with prevalence of collagen III and decrease in elastin compared with control. Impairment in elastic fibers quantity and distribution affects microcirculation and results in connective tissue desorganisation in uterine ligaments, leading to genital prolapse. Further investigations are needed to reveal diagnostic markers and women at risk.

Chronic pelvic inflammatory disease: controversial issues
Abstract
Pelvic inflammatory disease (PID) is one of the most common serious infection of nonpregnant women of reproductive age. Goals of the therapy must include the resolution of clinical signs, the eradication of pathogens from genital tract and the prevention of sequelae primarily including the development of chronic inflammation. The paper deals with the currennt problems and contraversial issues of chronic pelvic inflammatory disease. This review presents the results of domestic and foreign studies, and the own view of the issue, the prospects and possible solutions.
