Современная концепция патогенеза синдрома хронической тазовой боли, индуцированной аденомиозом

Аннотация

Синдром хронических тазовых болей является одной из актуальных проблем современной гинекологии. Одной из причин хронической тазовой боли (ХТБ) является аденомиоз. Формирование болевого синдрома при данном заболевании может быть обусловлено комбинацией трех механизмов: изменением нейрогуморальной регуляции; стимуляцией роста нервов и сосудов, воспалительным ремоделированием миометрия на фоне нарушения кровообращения и склерозирования сосудов, что в совокупности приводит к нарушению нейроиммунных отношений, предопределяющих повышение количества и сенситивности ноцицепторов на фоне хронического иммуновоспалительного процесса в эндометрии и миометрии. В связи с хронической тазовой болью данное заболевание является не только сложной медицинской, но и социально-экономической проблемой.
