Melatonin, a neuropeptide, is well-known to be deeply involved in the process of organism ageing. This hormone takes part in oxidative stress degree decreasing in ovary follicles and defends oocytes from aggression of free radicals. The aim of this study was to assess the outcome of traditional ART program with additional correction with exogenic melatonin in infertile women of advanced reproductive age. It was characteristic for infertile patients of advanced reproductive age to have sharp decrease in levels of melatonin secretion. This reflects disturbances of hypothalamo-pituitary chain of reproductive system. Additional donation of exogenic melatonin (Melaxen) in some cases of ART program potentially increases the chances of successful outcome.</abstract><trans-abstract xml:lang="ru">Эффективность экстракорпорального оплодотворения напрямую зависит от возраста пациентки. «Бедный» ответ яичников, ухудшение качества ооцитов, снижение качества получаемых из них эмбрионов, утрата ими способности к нидации в эндометрий препятствуют наступлению беременности. В то же время установлено, что большую роль в старении организма играет нейропептид мелатонин. Этот гормон, присутствующий в органах женского репродуктивного тракта, участвует в понижении степени окислительного стресса в фолликулах яичников и защищает ооциты от повреждающего действия свободных радикалов. Целью нашей работы явилось изучение результатов лечения бесплодия методами ВРТ с включением экзогенного мелатонина на основе данных уровня продукции мелатонина у женщин старшего репродуктивного возраста с диагностированным бесплодием. В результате нашего исследования обнаружено, что для женщин старшего репродуктивного возраста характерно резкое снижение секреции мелатонина, что является отражением расстройства гипоталамо-гипофизарного звена репродуктивной системы.
