Аbout typical responses of the damaged heart

Abstract

On the basis of experimental research we described nine typical (nonspecific) responses of the damaged heart which are initially protective and adaptive but are able to have a pathogenetic character under some certain conditions. These typical responses are as follows: shortening of the effective refractory period of the myocardium, cardiomyocyte mitochondrion swelling, activation of lysosomal apparatus, initial intensification of lipolysis, hypertrophy of cardiac muscle, formation of collagen muffs around blood vessels due to intoxication, functional deafferentation of the heart, changes in the intensity of cardiomyocyte apoptosis, rearrangement of cardiac chronostructure. The represented results can be used as a basis to elaborate some new effective methods of prophilaxis and treatment of heart diseases caused by different etiological factors.

Corrective effect of modified gas medium and temperature on functional status in animals after carotid glomectomy

Abstract

Carotid glomectomy is an adequate method for studying the functional role of peripheral chemoreceptors in the regulation of body functions. After glomectomy in animals the blood glucose level increases, respiratory rate decreases, oxygen consumption and carbon footprint signs of anaemia develops. The results suggest the possibility of physiological correction of the level of glycemia and gas exchange in animals during periodic action of hypoxia, hypo- or hyperthermia after glomectomy.

The system of microcirculation: clinical and morphological aspects of investigation

Abstract

Modern data about microcirculation disorders are presented. The methodical base of carrying out clinical-morphological and functional researches of microcirculation is discussed. The individual typological features of microcirculation caused by active and passive mechanisms of modulation of a tissue blood flow are revealed. The characteristics of different forms of microcirculation disorders (hyperemic, spastic, spastic-atonic, stagnat and stasic forms) are given. Classification of microcirculation disorders, including four degree of insufficiency of tissue blood flow is offered.

Effect of adenylate cyclases activator on the cytoskeleton reorganization in thrombin-induced endothelial barrier dysfunction

Abstract

This work executed on a model of endothelial monolayer in vitro, is devoted to the study of mechanisms leading to endothelial barrier dysfunction. The dysfunction inductor, thrombin, acts on endothelial cells through a specific receptor, triggering cascades of intracellular reactions (including cytoskeletal reorganization) resulting to increased of endothelial permeability. Thrombin-receptor interaction leads to dissociation of heterotrimeric G-protein subunits (Gi, Gq and G12/13), each of which is potentially capable to handle one or more possible ways of dysfunction development. In the present study we analyzed the role of Gi protein in the occurrence of cytoskeletal disturbances that lead to barrier dysfunction. According to our results, it can be assumed that the activity of Gi protein does not directly affect on the endothelial barrier properties.

In article the processes of intercellular adhesion in unchanged proliferating endometrium, hyperplasia and adenocarcinoma are surveyed. Expression E-kadherin, β-katenin and claudin-1 at different stages of epithelial neoplastic transformation of endometrium by immunohistochemical method had been estimated. The role of key proteins Wnt-signaling pathway and Claudine-1 in the development of malignant transformation of glandular cells of endometrium was established.

Abstract

The comparative analysis of the quantity of matrix metalloproteinases (MMP) 2,7 and 9 in breast cancer is carried out; the concentration of sex hormones receptors and protein HER-2/neu are determined. Authentically high values MMP-2 and MMP-7 are found in a tumor tissue in contrast to the normal one. The elevated concentration of ММР-2 in a tumor is connected with receptor status (p < 0,05). The highest values of concentration ММР-7 and ММP- 9 are found at RE-, RP- and HER-2/neu + tumors, and also in the tumors with the size more than 4,0 cm.

Abstract

26 pieces of corpses' lungs died from acute cerebral trauma had been investigated by morphological methods. The purpose of the research work is to estimate the character and time of morphological signs, which characterize acute lung injury. It was obtained that the most early morphological changes in lung - injuries of bronchial mucosa membrane, circulation damages (microcirculation), atelectasis, distelectasis and acute emphysema. Lung edema and signs of systemic inflammation reaction occur during the first hours after the trauma and aspiration.

Microalbuminuria in risk stratification of non-diabetic hypertesive patients

Abstract

Microalbuminuria (MAU) and a reduction in glomerular filtration rate are independent predictors of cardiovascular morbidity and mortality. Aim was to assess the role of MAU in risk stratification of non-diabetic hypertensive patients. The subgroup of patients with MAU was much more likely to show the other signs of subclinical organ damage as compared to patients with left ventricular hypertrophy, carotid abnormalities or increased pulse wave velocity. Thus, MAU is the integral marker of subclinical organ damage.

Influence of basal level of the general cholesterol on efficacy of antiviral therapy of the chronic hepatitis with peg-interferon-α 2B and ribavirin

Abstract

Chronic hepatitis С - one of the most widespread bloodborne infections in the world, which represents an important problem as one of the frequent causes of disability of young patients and indications to liver transplantation. One of the less studied, but extremely actual problems is eventual influence of the lipid exchange disturbances on the results of combined antiviral therapy. Influence of hypercholesterinemia on chronic HCV-infection and results of antiviral therapy are not studied enough and have certain scientific and practical value. 144 adults (> 18 years) with HCV-infection, who have passed their first course of antiviral therapy with PegIFNα-2b and ribavirin were surveyed. Low degree of fibrosis (measured by elastography, Fibroscan) or its absence was revealed in 77 patients while 67 patients had the significant fibrosis. Basal level of general cholesterol significantly did not differ in both groups. The level of general cholesterol was significantly above in convalescents despite of liver fibrosis degree. Patients with low fibrosis had significantly high level of general cholesterol in case of sustained viral response (SVR). Contrary in group of patients with the significant fibrosis the similar relations did not detected. Therefore, the baseline hypercholesterinemia is positive, irrespective of fibrosis degree, predictor of SVR.

Prevalence of ischemic heart disease among males with tuberculosis

Abstract

Three follow-up single-stage standardized examinations for revealing of ischemic heart disease (IHD) were conducted among males at the age 50-59 with tuberculosis in 1972, 1998, 2011 years. The results of studies are conformed that prevalence of IHD among males with tuberculosis is not changed during this period and remained on relatively high level from 21.0-31.9%. Practically every 5th male with tuberculosis has clinical signs of IHD that could be considered as a support of necessity of joint simultaneous treatment of tuberculosis and IHD.

Abstract

The work is based on the analysis of complex examination and treatment of 39 patients with chronic capitate pancreatitis. All patients with symptoms of obstructive jaundice underwent endoscopic intervention. In 10 (25.6%) cases pancreaticoduodenal resection (PDR) was performed, in 3 (7.7%) patients pancreaticojeunostomia (PJS) was imposed. Resection of the pancreatic head (RPH) was performed in 10 (25.6%) cases. Specific complications occurred only after the PDR and the PJS. The high level of mortality was registered after the PDR - 20%. Length of hospital stay was significantly lower after RPH. The quality of life in 28 (71.8%) patients in the late postoperative period was evaluated by the SF-36 Health Status Survey Questionnaire. The best results were registered in the patients who underwent RPH.

Long-term results of interlamellar keratoplasty for surgical correction of high myopia and astigmatism

Abstract

The Department of eye diseases and ophthalmology of Peoples' Friendship University of Russian has devised a method of the interlamellar circular, tunnel and sectoral keratoplasty for surgical correction of myopia and astigmatism of high degrees. In this article we have analyzed the results of our method. In total 140 operations were performed on 99 patients with follow-up period up to 13 years. 443 alloimplants were transplanted into the layers of the cornea and in 96.83% of cases (429 implants) transparent engraftment of the implants was achieved. Interlamellar keratoplasty is applied for correction of high myopia from 8.5 diopters up to 17.0 diopters; simple, complex myopic and mixed astigmatism from 3.5 diopters up to 9.5 diopters. The significant advantages of the interlamellar keratoplasty are: small trauma, lack of deep incisions of the cornea, the intact optical center of the cornea 6,0 mm, controllability of the refractive effect by replacing of alloimplants or their complete removal, if it is necessary, without any consequences for cornea.

Craniomandibular pains in orthopedic stomatology. Clinical features, diagnosnosis, treatment

Abstract

Depending on localization of pain source there are somatic pain (a source is in the bone-muscular system) and visceral, arising from the internal organs. If the pain is caused by irritation of pain receptors (without their damage), it is considered to be a nociceptic pain. In case of damage of pain system struсtures it belongs to a neuropatic pain. Craniomandibulary somatogenic pain which dentists most often face are mainly nociceptic. Results of the special research conducted by us on the study of the mechanism of action and efficacy of blockades show that particularly miofascial trigger points (MTP) are the main sources of craniomandibular pains, and that injection of NSAID in МТP is an effective and safe method of arresting pain in case of miofascial pain syndrome.

The teeth restoration by author's computer program may be done as by direct as well by indirect technology.

Oral health of children living in an orphanage

Abstract

The hygienic condition of an oral cavity, intensity and prevalence dental caries in children living in Asenevsky orphanage of the Kaluga region and without regular dental service is studied. 17 children aged from 4 until 18 years (9 boys and 8 girls) were examined. Obtained data revealed low level of oral hygiene, a high prevalence (94.1%) and intensity of dental caries (5,9%). Furthermore the analysis of stomatologic health of handicapped children of the Moscow children's boarding school № 28 for the disabled where the stomatologist works is carried out.
