Cardiovascular risk factors and diabetic foot syndrome in south indian diabetic population

Abstract

The diabetic foot syndrome is one of the most frequent reasons for hospitalization among the diabetic population and also one of the most common cause for lower-limb amputation. Apart from the obvious associated morbidity, patients with diabetic foot have an increased risk of death from cardiovascular diseases. Correction of glycaemia, serum lipids and blood pressure lead to a regress in progression of diabetes complications. In this article, the influence of cardiovascular risk factors on occurrence and progression of the diabetic foot syndrome in the Indian diabetic population during a 1-year follow up was analyzed.

Work is devoted to research dynamics of parameters of high resolution ECG (HRECG) and heart rate variability (HVR) methods in patients with acute coronary syndrome in determining myocardial electrical instability and disease trend the carried out. The researches have shown that, the HRECG and HRV variables in acute coronary syndrome, patients have various possible changes, which correlate with the severity of the current disease.

On rheological properties erythrocyre at sick of the arterial hypertension with dyslipidemia

Abstract

Purpose: comparative investigation of the influence of several inhibitors hydroxymethylglutaril coenzyme A-reductase - Fluvastatin, Lovastatin and Simvastatin on rheological properties of erythrocytes in patients with arterial hypertension and dyslipidemia. The total number of examined and treated patients was 95. Arterial hypertension 1-2 degrees, risk 3 and dyslipidemia II type (recommendation VNOK, 2007) was revealed in all patients The average age of patients was 52,4 ± 2,6 years. The control group consisted of 26 healthy people of similar age. Patients have been divided into 3 comparable, homogeneous groups and were administered either Fluvastatin or Lovastatin or Simvastatin for 12 months in each group. Prescription of Fluvastatin and Lovastatin to patients with arterial hypertension and dyslipidemia for 1 year may reduce the activity of lipid peroxidation in erythrocyte membranes, its deformation changes and aggregation activity whereas one-year therapy by Simvastatin allows the erythrocyte's activity to reach the values of the control group.

Cytokine regulation of immune response against bacterial and viral co-infection of gut, caused by h. pylori and herpesviridae

Abstract

104 patients with the inflammatory diseases of stomach and duodenum were investigated. Levels of the IL-1β, IL-2, IFN-γ in blood serum were detected by ELISA. For detection of Helicobacter pylori (H. pylori) and Herpes viridae in biopsies from the zone of inflammation polymerase chain reaction (PCR) was used. Expression of mRNA cytokines: Il-1β, Il-2, Il-4, Il-6, Il-10, Il-18, IFN-γ, TNF-α in biopsies from 37 patients was detected by real time PCR. Results of biopsies revealed high frequency of detection of pathogenic strains of H. pylori (80%) and Herpes viridae (EBV, HHV-6, HHV-7, HHV-8) in various combinations. The increased levels of IL-1β, IL-2, IFN-γ in serum and of mRNA cytokines in infected mucous membrane were observed.

Abstract

Chronic virus lesions of a liver are one of the principal causes of a growing mortality in the Russian Federation and other countries from a cirrhosis and a hepatocellular carcinoma. At the present day one of the most significant prognostic factors of decreasing efficacy of therapy HCV-infection is insulin resistance (IR). 114 patients, who were treated with Peginterferonα-2b plus Ribavirin within 2008-2010 yrs., were prospectively studied. Patients with IR, but with the normal carbohydrate metabolism (n = 59) have been divided into 2 groups: the 1st group (n = 22) was prescribed combination of Peginterferonα-2b (1,5 mcg/kg per week) and Ribavirin (15 mg/kg per day) + Metformin (20 mg/kg/per day), with their informed consent, the 2nd group (control) - Peginterferonα-2b (1,5 mcg/kg per week) plus Ribavirin (15 mg/kg per day) (n = 37). Thus, IR is specific feature of HCV, irrespective of presence or absence of adiposity. IR is revealed in 37% of patients with normal body weight. Besides the improvement of way of life, diet observance, medical correction of IR in patients on interferon therapy is necessary. Correction of IR by Metformin is reasonable, beginning from value of HOMA-IR ≥ 2, at any genotype HCV, and considerably improves results of Peginterferon α-2b plus Ribavirin therapy, especially in patients with low or normal body weight. Metformin as the third component of Peginterferon α-2b plus Ribavirin therapy is safe and effective. Medical correction of IR is a new aim in treatment of hepatitis C.

Algorithm of investigation for choosing repeated surgery on aorta and arteries of the lower extremities

Abstract

Aim: to estimate the diagnostic capabilities of ultrasound and angiographic methods in evaluation of the vascular bed in patients with occlusion and stenosis of arteries of lower extremities. The results of clinical investigation of 108 patients with relapse of chronic critical ischemia of lower extremities after reconstructive operations were analyzed. All patients were clinically and laboratory examined, the condition of the arterial system investigated by continuous wave Doppler (CW Doppler), color duplex scanning, X-ray angiography, magnetic resonance angiography, computed tomography, three-dimensional computed tomography in three-dimensional reconstruction. In 44 patients, an objective cause of thrombosis in re-development areas and relapse of chronic critical ischemia of lower limbs was detected. Based on the survey re Reconstructive surgery for each patient was elaborated. The optimal methods of preoperative diagnosis of complex lesions in lower limb arteries are Dopplerography, duplex scanning of main arteries, arteries of the foot, toes and angiography.

Diagnostic value of hormones concentracion in blood and follicular fluid during ovarian stimulation in IVF cycles

Abstract

We present data on concentrations of serum and follicular fluid hormones in IVF cycles in relation to the number of punctured follicles. Circulating periovulatory estradiol levels correlated positively with the number of punctured follicles. We observed significant decrease in circulating AMH concentrations in periovulatory period compared to baseline levels. Estradiol and progesterone concentrations in follicular fluid did not correlate with the number of follicles or serum levels of these hormones. AMH and inhibin B in follicular fluid were higher in patients with greater number of punctured follicles.

Treatment of the mixed bacterial and virial infections of the bottom department of sexual ways of women

Abstract

Comparative results of treatment of women with genital herpes against bacterial infection of the bottom departments of sexual ways from using three forms antivirus-immunomodulate preparation panavir are analysed: in the form of the rivers-talnyh suppositories, protective intravaginal gel of this preparation and there combination. It is established that clinical improvement of symptoms of disease were like at use of all forms of panavir. At the same time duration of relapse for sure decreased at patients with use rectal suppositories in 1,6 times, intravaginal gel - in 1,4 times, and at their combination - in 2,2 times. Simultaneously with it at appointment of all forms of preparation quality of life and initially suppressed indicators of immunity that proves inclusion panavir in practice of treatment of women with the mixed bakterial-virus infection was restored.

Efficacy of ozonetherapy in the complex treatment of psoriasis

Abstract

Aim: to study the efficacy of autotransfusion of ozone modified erythrocyte suspension in the complex therapy of extensive psoriasis. 118 patients with progressive stage of extensive psoriasis from the age of 18 to 66 have been investigated. Clinical picture, some indices of coagulogram, protein metabolism, lipid peroxidation and antioxidant protection processes in blood plasma have been studied in dynamics. Inclusion of ozonetherapy in the complex treatment of psoriasis contributed to the more rapid and complete positive dynamics of skin process, decreasing of seromucoid and С-reactive protein level, correction of some impaired coagulogram indices, normalization of the lipid peroxidation process level. The use of plasmapheresis with autotransfusion of ozone modified erythrocyte suspension appears to be the most effective. The tolerability of the procedures was good.

Correction of visual disturbances in patients with the primary open angle glaucoma by citicoline (CDP-Choline)

Abstract

Purpose: to estimate the efficacy of citicolines (CDP-choline) for treatment of glaucomatous optic neuropathy. According to the Innovative Educational Program at the Moscow Clinical Hospital № 12 40 patients (66 eyes) with primary open-angle glaucoma (POAG) stage II-III with normalized intraocular pressure were treated by citicoline in a dose 500mg/day during 10 days. The control group consisted of 40 patients with POAG stage II-III receiving an effective hypotensive therapy. We have evaluated the effect of citicoline (CDP-choline) on visual function, morphometric characteristic of retina tomography, characterizing the optic disk, and perimetric indexes of the static automatic perimetry and have compared the dynamic of estimated characteristics with those before treatment. In the group of patients, receiving citicoline (CDP-choline), visual functions have increased and perimetric indices of the static automatic perimetry have improved. Inclusion of citicoline (CDP-choline) in the treatment regimen of glaucomatous optic neuropathy makes standard therapy more effective for most indicators.

The work is devoted to studying of the functional state of the autonomic nervous system (ANS) of HIV infected patients. Heart rate variability, frequency and severity of vegetative manifestations. were studied in 103 HIV-positive patients. The control group consisted of 23 healthy persons. Obtained data revealed changes of the functional state of ANS along with development of hypersympathicotonia and increasing of frequency and severity of vegetative manifestations as the HIV-infection progressed.

Long-term outcomes of surgical treatment of thyroid nodular formations

Abstract

Aim: to study long-term outcomes of treatment of nodular goiter, depending on the initial pathology and the volume of surgical intervention. In the late postoperative period ultrasound changes in thyroid tissue after subtotal resection of both lobes observed in more than half the patients. Adequate replacement therapy can maintain euthyroid status of patients, even after thyroidectomy, and postoperative hypothyroidism can not be regarded as a complication, as well as a logical outcome of major resection of thyroid tissue.
